Private Care Management

Care Management, or Case Management as it is also referred to, is a collaborative process of assessment, planning, facilitation, care coordination, evaluation and advocacy for options and services to meet an individual’s and family’s comprehensive health needs through communication and available resources to promote patient safety, quality of care, and cost effective outcomes.

As a privately owned company we provide care management and guidance without limits to help you navigate the system when dealing with aging and care issues. Our private care management services can be integrated at any time- when services are initiated or whenever needed on an hourly basis.

Families that prefer to have the peace of mind that comes with consistent high level care coordination and services can join our Concierge Care Management Program, inclusive of five hours of professional care management for $475 each month.

The Home Care of La Jolla Approach

Home Care of La Jolla's care managers are licensed nurses working under the supervision of Dr. Patricia Lutfy, MD. Our case managers provide an elevated level of care and service to clients that wish to have individualized service that encompasses all that is entailed in caring for a family member and ensuring the best possible quality of life for all involved. We relieve the family of the stressful duties involved in managing the various services and medical care needed. Our care managers also provide professional advice about Home Care, Assisted Living, Caregiving, Elder Care, and Senior Care based on their assessment, interviews with caregivers and discussions with a client's medical team and doctors.

We understand the stress and challenges faced by seniors, caregivers, and their families. We are able to make things easier for individuals and families who struggle to find the time, energy and resources to balance their family needs, caregiving responsibilities and work.

Our team are experienced professionals that can help assess the particular needs of a family, the level of care needed, and the living situation. Help them navigate the medical system and planning and supervising care, among other services.

The Philosophy of Care Management

The underlying premise of private care management is based in the fact that when an individual reaches the optimum level of wellness and functional capability, everyone benefits: the individuals being served, their support systems, the health care delivery systems and the various reimbursement sources.

Care management serves as a means for achieving client wellness and autonomy through advocacy, communication, education, identification of service resources and service facilitation. The care manager helps identify appropriate providers and facilities throughout the continuum of services, while ensuring that available resources are being used in a timely and cost-effective manner in order to obtain optimum value for both the client and the reimbursement source.

Care management services are best offered in a climate that allows direct communication between the care manager, the client, and appropriate service personnel, in order to optimize the outcome for all concerned.

Private Care Management Rates:

Licensed Care Manager: $135/hour; Licensed Care Manager Concierge Program: $475/month

The Care Manager Concierge Program includes  up to 5 hours** of the following services:

Two house calls per month by a licensed nurse or Nurse Practitioner to review and assess the client care plan and needs. Care plan updating coordination with caregivers and other providers. Continuing education of caregivers and other providers and integration of new or amended services. Advocating for the client as needed by speaking directly with doctors, insurance companies and any ancillary service providers including Home Health and Hospice. Communication with family members to inform them of changes, available services, progress reporting and address questions and concerns. Conscripting new services and managing coordination of care and services as requested by the family.

All Home Care Of La Jolla nurses, caregivers and aides are employees of the company, educated, vetted, bonded and insured. All caregivers are registered with the state caregiver registry having met the qualifications necessary to provide in home care in compliance with state and federal laws. All nurses have current licensure in good standing and passed our stringent testing and reference check processes.
